Amanecer Counseling & Resource Center
Amanecer Counseling & Resource Center (ACRC) is a nonprofit organization providing bilingual, culturally responsive, and trauma-informed behavioral health services, legal advocacy, and community resource coordination to individuals and families across Delaware.
Healthy Village Resources
With a strong focus on underserved communities—particularly Hispanic and immigrant populations—Amanecer works to ensure equitable access to care, healing, and support.
Key services:
- Mental health therapy – general mental health support and counseling
- Specialized services for victims of crime – legal advocacy, coordination of legal resources, assistance navigating complex legal systems, and support in understanding legal rights and accessing protections.
- Bilingual community resource navigation – Help connecting to essential services such as housing, health care, public benefits and other community resources

Cornerstone West CDC & West End Neighborhood House
Located in Wilmington’s West Side, these organizations collaborate to strengthen the community through housing development, economic growth, education, employment, and family support. West End Neighborhood House provides outcomes-driven programs that help families achieve self-sufficiency. Cornerstone West CDC leads neighborhood revitalization efforts as part of the West Side Grows Together plan. Together, they offer resources and advocacy to ensure residents and communities thrive.

Healthy Village Equity Center
The Healthy Village Equity Center is intended to address the social needs of the community and the patients at Saint Francis Hospital. Located in the former hospital gift shop, the Healthy Village Equity Center is a service comprised of:
- Tiny Steps
- Car seat safety
- Home safety
- Safe sleeping
- Diapers, formula, baby essentials
- Everyday essentials
- The CARES program
- Emergency meal program
- Surplus food program
Social need screenings are available.

Hope Commission Women’s Reentry
Hope Commission Women’s Reentry is Delaware’s premier reentry program providing cognitive behavioral interventions and referrals to volunteers referred from the Delaware Department of Corrections and Baylor Women’s Correctional Facility. Services include a GED program in partnership with West End Neighborhood House and one-on-one and group sessions with the reentry coordinator to help navigate other support systems within the community.
Limen Recovery + Wellness
Limen Recovery + Wellness is a mission-driven organization providing compassionate, comprehensive support for individuals navigating substance use and co-occurring mental health disorders. Through a holistic continuum of care, Limen offers outpatient and residential treatment, sober living options, peer-led recovery services and trauma-informed wellness programming – delivering integrated, person-centered care that promotes dignity, healing and sustainable recovery.
Key services:
- Outpatient and intensive outpatient programs: Flexible, clinically guided therapy that allows patients to continue daily responsibilities while engaging in treatment.
- Residential treatment program: 24/7 care in a structured, healing environment focused on stabilization and recovery.
- Sober living residences: Safe, community-based housing to support long-term recovery.
- Peer recovery support services: One-on-one guidance from certified recovery specialists with lived experience.
- Care navigation and case management: Assistance with accessing essential services like health care, housing, employment and legal aid.
- Hospital-based engagement: On-site peer support and coordinated follow-up to reduce gaps in care and improve recovery outcomes.
- Wellness and life skills programming: Tools and resources focused on mindfulness, nutrition and financial literacy to promote resilience and independence.
Merakey
Merakey’s Wellness Recovery Team is an integrated health care service that works to improve the physical and behavioral health of individuals with serious mental illness, substance use disorders, and physical health conditions.
Each team member will work with patients to:
- Improve understanding of medication.
- Improve self-management and wellness strategies, including, but not limited to stress management, diet/nutrition, exercise and use of preventative care.
- Develop improved relationship skills.
- Increase confidence in relating to physicians of all types and understand how to negotiate and communicate with all health care providers.
- Increase coordination of care.
- Increase advocacy for one’s own health.
